Why is My Missing Conversion Request Rejected?

Possible reasons for this are (but not limited to):

1. Reported missing conversion immediately but there was a delay for the offer and it is "Manually Tracked".
2. Duplicate missing conversion report has been made for the same conversion.
3. Wrongly submitted information, such as Order ID.
